Long-Term Residency

Long-Term Residency Program Overview	This program is designed to assist senior citizens and individuals with cognitive and physical disabilities. It provides supervision for the clients' daily needs either through staff or outside nursing services. It also provides a safe, "home-like" atmosphere with around the clock supervision.

Mobile Medical Unit Overview	This program is provided by Healthreach for the Homeless. The program seeks to provide basic medical care to those individuals that require it. The Mobile Medical Unit also provides service hook-ups for those individuals in need. The Open Door Mission is used as a Mobile Medical Care Site.

Hours of Operation	Every Monday & Thursday, from 4:00 pm to 6:00 pm
Eligibility	This program is available to anyone in need of medical attention

Representative Payee Program Overview	The Open Door Mission's Representative Payee Program is a personal, confidential financial management program approved by the Social Security Administration.
Location	Open Door Mission Caring Center 156 N. Plymouth Ave Rochester, New York 14608
Hours of Operation	• This program is available every Monday-Friday by appointment only. • The program operates from 9:00am-5:00pm.
Eligibility	Any individuals with case managers who need assistance in managing their monies. Individuals that receive Social Security, supplemental security income, or other financial assistance are eligible.
